To submit a Public Information request, applicants are required to fulfill the following requirements:

• Complete the provided information request form.
• Provide clear and complete personal identification details.
• Attach a copy of a valid identification document (ID Card/Family Card/Driver’s License/Student Card).
• Provide a clear residential address and contact phone number.
• Clearly specify the type of information requested.
• Explain the purpose and objective of the information request.

Attach an assignment letter or power of attorney if the request is submitted on behalf of an institution or legal entity.
